Steps to enroll:
- Read the instructions below.
- Complete Enrollment Agreement and School Policies.
- Submit a copy of a current government picture ID and transcript.
- Sign the School Policies form electronically or on a downloaded pdf, mailed or scanned and emailed.
- Purchase required books from one of the textbook sources.
Below are the links to the Enrollment Agreement, HSI-School Policies and a list of required books and book suppliers. Please carefully read the agreement and the policies and send the signed documents to the school. You can enroll at any time.
Enroll using our online form: Read, fill out and sign the Enrollment Agreement and the School Policies. Download and print out copies of the required and recommended book list and book supplier list. Review tuition costs on the tuition and fees page.
If you are enrolling or transferring into Advanced Clinical Studies, please use the ACS Enrollment Agreement.
